The letter went on to say the clinic is not able to confirm if they had been immunized with AstraZeneca or Moderna. The letter goes on to say if those who received the letter experience any "rare but serious signs or symptoms" within 42 days, they should go to the nearest emergency department or health centre for immediate assessment.“Our clinical team was notified by NorWest immediately after NorWest realized the error. The clinical team reviewed the safety and effectiveness implications of the situation," a spokesperson for the province told CTV News in a statement.
"We apologized for any worry this may have caused them hearing this news and have taken appropriate steps to ensure this will not occur again," she said in an email to CTV News.
Before to you administer any drug let it be paracetamol to the vaccine. You check name/ dob/ dose/Etc. So there shouldn’t be any room for mistakes.

I had AstraZeneca and Pfizer and am alive and very well. I know a lot of people who did the same and no one grew a third eye. The panic is caused my the misinformation reported my media to whip up fear and mistrust. This constant flip flopping on what’s safe needs to end.

Why don’t they know who received AZ? Each shot comes from identifiable vials. Ontario receipts state the type of vaccine and batch number. Is this another half assed BrianPallister job?

That's not accidental. Each vile has to be identified before clinic day. On clinic day, each nurse administers the vaccine must inspect the vial and note it in the system. Too many eyes & hands saw those vials for it to be error. It was a choice.
Like it really matters, a vaccine is a vaccine just be glad you got it.

Happened to a trucker friend went to moderna vac clinic here in Ontario only to receive a letter a few days later saying that he got the AZ jab. His mother, a nurse is furious with the clinic for giving him AZ ,he's 35 and l don't think they changed the jab for people under 50.
As a Pharmacy Assistant (and not speaking on behalf of my employer), I fail to see how this could even occur. The vials and preparation between AZ and Moderna are very different, unless the facility was administering both vaccines at the same time. Quite worrisome!
